Excerpts from Guide for Aviation Medical Examiners Applicant History
All applicants must be asked to show proof of age and identity. On occasion, individuals have attempted to be examined under a false name. If the applicant is unknown to the AME, the AME should request evidence of positive identification. A Government issued photo identification (e.g., driver’s license, identification card issued by a driver’s license authority, military identification, or passport) provides age and identity and is preferred. |
