Applicant History - Item 18. Medical History b. Dizziness or fainting spells.
The applicant should describe characteristics of the episode such as spinning (vertigo) or lightheadedness, frequency. State any identifiable factors leading up to and surrounding the episode (before, during or after). Report any associated neurologic symptoms such as headache, nausea,
interruptions of conciousness, or paresthesias (tingling or other unusual sensations). Include diagnostic workup and treatment if any. Be prepared to discuss effectiveness and any side effects from the treatment.
